A vaccination ticket is required to receive a COVID-19 vaccine.
Foreign nationals who have entered Japan for a short stay and have renewed their period of stay and wish to be vaccinated should apply for the issuance of a vaccination ticket. Applications can only be made by postal mail. It takes about 10 business days from the time of application until the vaccination ticket is delivered to you.

Documents to Be Attached:
・A copy of an ID (e.g. Passport) of the person to be vaccinated
*If you cannot attach a copy of any ID, please indicate the reason why on the application form.
・A copy of a document verifying your residence (e.g. utility bill receipt, rental housing contract, or certificate of accommodation)
・A copy of a document confirming your first and second vaccinations (e.g.a copy of COVID-19 vaccination certificate) *Only for those who wish to receive a 3rd COVID-19 booster shot
・A copy of a document confirming the third vaccination (e.g. a copy of COVID-19 vaccination certificate) *Only for those who wish to receive a 4th COVID-19 booster shot
・A copy of the proxy’s ID *Only when the application is made by a proxy
